I formally request that the following inaccurate items be immediately removed from my credit report.  If an investigation on your part is required to satisfy my request, please initiate the action as quickly as possible.  Under the provisions of 15USC, Section 1681i of the Fair Credit Reporting Act, I demand that these items be re-verified and deleted from my credit report.  The enclosed documentation proves that the judgement entry should not have been made on my credit report.  The specific error in question is as follows:
